I understand as an employee of Geiger Logging, Inc., l am not allowed to use a hand-held cell phone while driving and will refrain from doing so while on company time or in a company vehicle. Only hands-free use of a cell phone is allowed (i.e. Bluetooth)

I understand as an employee of Geiger Logging, Inc., I am not allowed to be under the influence of, or in possession of drugs, or alcohol while in a company vehicle or on company time.
I understand as an employee of Geiger Logging, Inc., I am not allowed to have any non­employee passengers in a company vehicle. I am also not allowed to use any vehicle for non­business use without prior written consent.
I understand as an employee of Geiger Logging, Inc I must use safety belts at all times while driving or riding in any company vehicle on company or personal business, or in any other vehicle while on official company business. I understand this policy applies to all employees and all occupants of vehicles driven by employees on official business, whether in company owned vehicles (including trucks), rented vehicles, or employees' personal vehicles. I understand safety belt use will be enforced in the same manner and with the same consequences as other work rules. Disciplinary actions may range from ________ for a first offense to ________ for chronic offenders.
I acknowledge that I have received a written copy of the Zero-Tolerance Letter, that I fully understand the terms of this policy, that I agree to abide by these terms, and that I am willing to accept the consequences of failing to follow the policy.
